TB test results are only valid for one year. The expiration date of the TB test must be after the last day of the semester for which you attend. Available at Gila County Health Dept. Provide results by start of Skill Labs. |

CPR |
Must obtain or have a current CPR card effective through the end of the course - Health Care Provider Level “C” from AHA. Classes available at Gila Community College, local hospital, fire department or Payson Care Center. No Online courses permitted. Provide card by start of Skill Labs. |

MMR and Hepatitis B |
Students must provide proof of MMR and Hepatitis B vaccinations or sign declination. Provide proof by start of Skill Labs. |

Attendance |
Must attend 65 hours of lecture, 30 hours of Skill Lab and 42 hours of Clinicals to remain in the program. |
